I'm pretty sure 38x15.50's won't fit.

I had a toyo open country M/T 38x15.5 on a 18x10 -25 with 4.50 backspacing and had rubbing on the frame but they look much more aggresive then a thinner tire.

Just as a point of reference, several members have fit 38x15.50's on 97-01 4x4's by using a 3 inch body lift, cranking the torsion bars all the way up and using a 10 inch wide wheel with 4.5 inches of bs, but they still had some rubbing. I think the 12 inch wide wheel is gonna cause you problems.

thats basically my truck and thats exactly what i said....here are your options
1. Let it rub (harder to get into parking spots and maybe not that safe)
2. Use spacer (they add a new vulnerability and I would never)
3. Go with the 13.5

Its your decision!

So out of curiosity what gears?
