I wonder if a floor hump out of a manual Super Duty will work? It's not that hard to swap out the floor humps, just a few spot welds to drill out. You've gotta swap them on Super Dutys too when you go from the auto 4r100 to manual. I'm sure it'd be a little harder on an F-150, but it's not like the welds have to be perfect, nobody will ever see them.

I have looked into this in depth for my 05' f150, and it will take some work, but like anything, can be done. You will need a body lift (3" +), a ZF 5 or 6 from a 99+ F250/350 (prefered) that has the 5.4L or the 6.8L gas engine, you might as well get the transfer case also. Find a F150 in your body style that came with a manual trans. and get the clutch stuff, like the pedals, shifter, master cyl. and the hardlines. Custom drive shaft with also need to be made I assume. All in all you are looking at about 2k to convert. Best bet is to get on some Ford diesel forums and look for guys that are parting out their Z F5 stuff to put in a ZF 6, or hit up a yard and pull all the parts yourself.
I am still entertaining the idea, but work over seas and do not have the time to do it right now.

Or - you can put a M5OD into it and treat it gently. It would HAVE to be out of a 97-03 with a 4.6, and matching drive configuration (2wd or 4wd). If your 5.4 is non-PI and stock, it should live for a while.
Yep, that's JMC. His 5.4 is supercharged.
There is a guy here with an M5OD, but his fifth one I believe.

1. Get a super duty with a manual... if you want a gasser, get the v10 with a manual, but they are hard to come by.
2. Go with a ZF 5/6, and stay away from the M5OD, unless you can find someone willing to fabricate stiffened parts to beef it up, but that might cost too much or else you might not be able to find someone. Unless you are easy with it. General driving will be ok to the M5OD, but don't expect to try to continually roast tires, drag race, etc with it, or it will break for sure.

If you have a '98 and it isn't falling apart, you probably aren't going to go through M5ODs like stuff-through-a-goose. I'd say try to find a low millage wrecked truck and pull everything out of there. You'll still have to get the drive shaft length adjusted, and you computer reflashed but otherwise you're there. There's a lot of info on how to do this posted.
The ZF5/6 would be great, but it will be harder and more expensive.
A 250/350 might be great, but also pricier and it's not for everyone. For instance, my driveway is so narrow, that I can hit both my house and the neighbor's backing out if I'm not careful.
The T56 would be amazing, but it's not a truck tranny and so few have done it that you wouldn't know the challenges before you got into it. So if you need your truck back on the road quick, you could get yourself in trouble.
The best advice is probably find a truck with the M5OD and one with the ZF5/6 and test drive them. If you like how the M5OD shifts, do it, if the ZF seems much better, you can afford it and you're up for the challenge, do it.
