Redneck trailer supply sells suspension components like a leaf spring perch. It would need to be welded to the frame by someone though. Make sure the frame is still strong enough before you attempt this route. If the suspension is rusting off, the frame is probably weak too. It might be time to scrap the truck.
You will need to know the spring width and the spring shackle bolt diameter. If a shackle is all you need, just about any part store should have or be able to get you that. At least any 4x4 parts store.

i had the same problem on my 94 ,first the dealer should be able to get them i dont think they were to much 20 to 25$ a piece i think ? now do not weld them on ,get some grade 8 bolts and replace the factory rivets and bolt them to the frame and add some high strength lock tight to the nuts,i have had mine on for 3 yrs with no problem.the one thing i did to the new ones is coat the(cup) part of the spring perch with under coating and drilled a small drain hole to allow water to drip out of the perch it seams that dirt and water collect there and they rot out.right now my truck is in the middle of a frame up resto in my back garage...hope this helps
