Yes, the handle has to come off. But that takes all of 30 seconds, once the door panel is off. it would be darn near impossible to do the mod, with the handle in place.

 

it looks good, but just how much light do you need at night?

 

They produce just enough light to enable you to see where the door handle is located, its not so much that it is distracting, and they are powered from the dimmer ciruit, so they do dim down with the instrument lights.
