There are lots of books on the subject of handshakes. But here's the business standard that seems to work very well...
The man's hand should be extended, open with the palm up. This is called the "inferior" handshake position - this allows the woman to meet you from the "superior" position.
Men generally meet as equals, palms roughly perpendicular to the floor. In a negotiation, you normally want to meet your "foe" as the superior, so you would extend your hand palm down, forcing them into the inferior position.
Of course, like everything else in the world, the next shrink will tell you something opposite to this. And then there is meeting in a social setting...

Interesting, most people pay little attention to other people shaking hands. Did she show you how she thought you should shake?
If you really want to upset her... shake by taking the lady's fingers (palm down) with your fingers, perpendicular to the ground and sideways between you and the lady, your thumb lightly on the back of her fingers. Bow down and touch your lips gently to the back of the lady's knuckles... the proper hand kiss for polite society. Melts the lady and pisses off all her friends.

How about this?
When she extendeds her hand, you grab her arm around the wrist with both of your hands, and start shaking it violently, like you are trying to shake her hand right off the end of her arm.
Always freaks them out a little.
