This has got deadlocked SQL query written all over it. Without actually looking at the code, 9 times out of 10 when these sort of apps start to hose up when you increase the load, SQL statements start tripping over each other.

You've likely hit the nail on the head. However, the double and triple posts themselves are the result of user impatience, the server hangs because MySQL locks, and the user hits the submit button several times. The cause of the multiple posts is known, the cause of the MySQL locking problems is not...yet.
