Thoughts on buying this 93 Lightning
Talked the seller down to 4500. My concerns are the tranny at 213k miles, hes the 3rd owner, he told me he has towed his car with it which I didnt think using this truck for towing was smart. Thoughts??

I traded my 94 F150 4x4 SWB for a new 2000 because it was noisy on the road and the pseudo twin I-beam front suspension cupped the tires no matter what.
You could put it on a lift to check out the rust before you buy it I presume. You should be able to find evidence of rust when you do that.
Being from the south, I wouldn't buy a vehicle that was owned in the rust zones.

my issue is the sellers blatant dishonesty and the fact it's probably rusted out underneath

The rust on the roof looks to be just a scratch that has gone down through the primer coat and is rusting...you could get someone who does spot repairs to fix it for probably $100. The undercarriage looks great for a 22 year old truck with 200K+ miles on it. I've seen 10 year old trucks that looked much worse...if you can get it for the price you're looking for I'd say you found a decent deal.
