18" spare tire with 20" wheels
I have a 2010 F150 Lariat with 20" wheels.

I noticed the spare was a 18".

I do not have a 4WD. Will there be an issue?

No. You will be fine. Lower the spare and pull it out, and pull one wheel off, they're the same height.
