Brake Problem?

I have a 2004 F-150 FX4. When hitting my brake to slow down, at 45mph-50mph, my truck shakes and shimmies pretty bad, is this the brakes/rotors, or maybe bearings or wheel alignment??? Any help would be appreciated.....

Sounds like warped rotors............

Do you feel it mostly in your seat or the steering wheel.
Seat...back rotors
Wheel....front rotors

An alignment problem will not cause shaking....bad tires could but it would shimmy only at certain speeds. The suspension might be a culprit but you stated that it only happens when you hit the brakes which rules out the suspension.
