Cam Positioning tool (holder)

I need to remove the right side head on 2001 5.4l. My Haynes manual says if the cam holder isn't used, damage to the valves or pistons my result due to valve spring pressure. I would only know by removing the tension on the chain... but if I saw it rotate it would be too late. Does the cam remain stationary with slack in the chain? or does valve spring pressure rotate it to some other position as the chain is loosened? Thank you for valued input.
