I just ejected a plug!
I probably havent gone 12k. My old van, the E-350 that had a 5.4 in it I took to the Ford dealer and had the plugs changed a week later it blew a plug. I dunno. Ford seems o have had a thing for not engineering good spark plug holes for awhile. Not that they are the only ones with this issue but still. They should have fixed the spark plug holes faster both in the 2v and the 3v. Took them over 5 years to add more threads to the 2v engine and nearly 5 years to fix the poor plug chamber in the 3v.
I got it done. installed the insert put everything back together it ran rough for just long enough to scare the crap out of me and smoothed out and it seems fine although I have yet to take it out of my driveway.
I know why this happened now. I took out the other coils and checked the torque of the plugs and they all were tight so I put all the coils back on including the blown out cylinder and I notice that cylinders coil is super loose feels like the bolt isnt tight so I check it out and the bolt is tight but its not going all the way down, not holding the coil. So what happened is the coil was moving around and it slowly broke the plug loose. Got some washers 3 of them and still had to tighten the bolt to the point I was worried I might break the bolt or strip the hole out but finally it held the coil.

I bet that is what happened..
But, when you washer the bolt like that, you have to be real careful. The coil ear will fracture then break completely off.
I'd keep an eye on it jethat, If it busts off, your pretty much screwed as far as the coil. You may be able to zip tie it, if it does break.
Iduno, I'd just fix the thread chamber. The metric taps , even a little set of metrics is pretty cheap to purchase. I'd be surprised if the washers secure it good enough, it might.

Glad the plug chamber fix went well, -I may have to borrow that from yuh someday.

I have a few spare coils so I'm not to worried about it. Next time I do plugs and all I'll get a tap or just get what evers in the hole out of there. I'm sure thats the deal. Took it for a test drive and it runs perfect.
Great! Congrats .
