Exclamation Extremely Rough Idle - Codes

107,500 miles, 5.4. This only seems to happen when it is hot outside or after highway driving regardless of the temperature. Once I come to a stop the truck starts knocking and running rough, not a slight knock, but a loud diesel engine knock. I can put it in neutral and keep it at around 1k RPM's and it quits. It will not do it in the morning during my short 3 mile drive to work, or back home for lunch, however on my trip back to work after lunch it does it. These are the codes Auto Zone pulled: P0340, P0345, P0452 P0012. I have replaced both VCT sensors as well as position sensors (twice) in the past. I recently did a plug change around 3k miles ago and it ran get until recently. Any ideas or thoughts are greatly appreciated, and thanks in advance for any responses.

Did you replace the VCT solenoids? Do you have much sludge? Both will cause the symptoms you describe as well.

Jack - I have replaced the solenoids, not sure how long ago. Sludge may be an issue, any ideas on how to clean the sludge if there is any? thanks.
