5.4 Multitude of issues

So here goes... I have an 01 SCREW with a 5.4 that has a nice 208,000 miles or so on it, back in October of last year I had a COP go bad #5 I believe, so knowing the milage and anticipating the rest going bad I had them all replaced. At the same time I had the plugs changed as well.. Problem solved or so I thought.. In the last month I had not 1, not 2 but 3 plugs blew out of the heads, breaking the COP at the bolt and making a loud ride home for about 5 minutes, each time I had to sent it to a garage and have a time sert put in and replace the COP.. Kicking myself for not doing it sooner.. I pulled the remaining COP's and checked the plugs, found 2 more that were litterally finger tight. Tightened the rest to the 20ft/lbs and back on the road..
Yesterday the truck started having a serge and sounded almost like an exhaust leak.. No problems ideling in park just under a load.. I dont know if another plug is about to go or something else is up.. Code p1405 popped up and Im not sure what it is.. of course the code cleared after reading it still having the same problem..
Any thoughts or opinions are welcome.. Thanks

Thanks, I will be looking into it more.. Is there something I should be looking for exactly.. I am not a complete idiot but A mechanic I am not..

You need to torque those plugs to 28 ft/lb, and do NOT use antiseize. You should also use only Motorcraft plugs. Whoever changed your plugs did not do the job properly.

Printed off the PDF of the diagram found the hose and sure enough it was off, I must have knocked if off while pulling the COP's. It' s back on and fingers are crossed that it was the solution..

Good deal. Hope it works. You do need to retorque all the plugs, though.

That will be my project for the week, I don't believe they used antiseize but I will ask tomorrow, I think I'm better off taking things in my own hands, never a better situation than being able to blame noone but yourself.. I didn't mess with the plugs that were replaced with the timeserts should I? I'm not sure if I should be wrenching on them seeing as they were a problem once already...

I'd still crack them loose 1/4 turn and retorque them with a torque wrench.
