cam phasers

What will happe if I don't replace the cam phaser in my motor. I have a few options so here goes
1. Replace cam phaser for 1/2 price. ~$400
2. live with it.
3. buy an extended warrenty ~$900.

I have had this truck for about a week and this is what the dealer offered.

Best case scenario - it'll be noisy forever.

Worst case scenario - the phaser could potentially get stuck in a full retard or full advance (or anywhere in between) and affect the running of the engine.

If it does lock up will it shut down or damage the engine.

No, it'll just run rough.
