Did you ever check those vacuum hoses on the cruise control, assuming your truck has cruise control? Your problems sound so much like the ones I had with a 1988 F-250. Don't just glance at the hoses - pull them around and unbend them to see if there are holes, slices, etc. where they bend. And, there is one that routes through a bunch of other wires attached to the firewall. Mine had rubbed holes, slices there too.

I repaired mine with black RTV silicone until I could get them replaced. The Ford dealer was a bit **** about replacing those hoses with the exact Ford replacement - they are thick and kind of hard to find that thick with the correct inner diameter. The dealer seemed to think the right hose mattered.
