Since the truck has had a lean condition for a year what does this do to spark plugs?

You don't have to remove the regulator, you only have to pull the vacuum line off it at either end to do the test. If it's that hard to get at I wouldn't even bother as long as you have at least 28 psi running at all rpm's. If it were easy to get at as it is on the V8's the test is easy.

but if it’s bad I’m going to have to replace it.

Truck was running earlier I just screwed harbor freight gauge on and it shows 0
I turn key on engine off 31 psi
i start truck 28 psi at idle
I rev up it goes to about 33 psi
engine running disconnect vacuum to regulator and it jumps to about 36 psi
plug it back in and back to around 28 psi at idle

recently replaced fuel pump and filter this past 7 days so regulator is good right?

Regulator is good. Fuel system is not your issue unless your injectors are dirty. Throw a bottle or two of Techron in the gas tank.

That should be okay, put the Techron in the gas.

last weekend when it wouldn’t start it was at 15 psi then all of a sudden it started. Since then I changed pump, filter spark plugs and pcv valve.

How big of a deal is replacing injectors on 4.2 f150
It started shuttering tonight and service engine light was flashing. I have po302 misfire code , po460 fuel level sensor a circuit and po455 Evan emissions leak detected gross leak no flow

Changed fuel pump unit and filter and plugs a few months back
drove 20 miles and on the way back it started acting up at first thought transmission was making noise but it was skipping or misfiring and service engine light was flashing.

Do you have to remove intake to replace injectors?
