possible slipping??

possible slipping??

well i'm new to this forum and new to having a truck. it's a 1995 f150 reg cab 300 I6 with 5spd manual transmission. Well i was driving today, and realized that when i start to go up a hill, i push down on the gas and the truck revs high then comes down and starts pulling, if you keep even pressure on the throttle, it will do this repeatedly, rev up then start pulling then rev up. Also happens when you are goin around 40mph in 4th then go to speed up without shifting to 3rd. This only happens in 4th and 5th gear. starts slipping then stops then will start slipping. What should i check and what could be some possible problems???
