shake/wobble

Hi, I have a 1995 Ford F150 XLT with 33x12.5x15 tires with a 4" lift, when I get to 40-45 mph I feel a vibration in the steering, wheel then a shake or wobble happens to the point that I have to stop to get it to go away. Here of late it does it when I hit a bump. Please can someone help
