Rotating Tires
I quit rotating my tires, everytime they were rotated they had to go in for roadforce balancing otherwise the steering wheel would shake. Just keep the tires inflated, correct alignment and don't mess with it. This has worked for me since my first set of aftermarket rims in 1998 on a new z71 (two sets of BFG's in 100k miles), 2002 z71 (two sets of BFG's in 92K miles) and 2005 fx4 (55K from stockers). Things might be different if I only drove in the city, but I drive mostly highway and alot of gravel.

I do it based on wear. I just keep an eye on them. If they look like they need rotating, I tend to rotate them. Max I'll let them go is about 10k. Heck, I forgot to rotate (yes, forgot) my Goodyear Conquests for like 25k and it still rode fine. I seriously beat up the tires from doing that though. As a matter of fact, the tread difference from front to back was a lot. The fronts were decent... but the rears were horrible.
Every 5,000 - 6,000 miles. When I bought my truck new, I purchased road hazard tire insurance from Discount Tire, which I also had included lifetime rotate and balance. So every 5,000 miles or so, I bring my truck in for rotate and balance, free flat repairs, and complete tire replacements for free if they cannot fix it.
I had my last truck 80,000 miles, very rarely rotated them if ever, the Goodyear's still had plenty of tread when I traded it in. This truck I did the same, rarely. Got some new BFG's at 68K only because I split a sidewall on a curb one night.
A friend told me he had the same BFG's and to make sure they get rotated or I'd have some problems. Bought these at Discount so they rotate and balance every 6K for free. Just had them done for the first time...
