greasing slip yoke

Question greasing slip yoke

I have noticed a clunk noise when I go from reverse to drive and sometimes from starting off at lights. I did a search and figured I would start off by greasing my slip yoke. Now I know the slip yoke is the piece coming out of the transfer case, but where does it need to be greased at? Does it need to be pulled out of the transfer case to be done? Also should I go ahead and replace my u-joints while I am at it? Thanks for the help in advance. This place is great. My pickup is a 1999 5.4L Sport 4X4 with 59,000 miles.
