Rear axle leaking fluid

I have a 97 4X4 4.6L 135K miles. The vent hose on the rear axle is leaking fluid. What causes this and is it hard to fix? Thanks for any help!

Have you driven in deep water where it could have sucked in water. Other than that the only reason I could see diff fluid coming out of the vent tube is heat.
