ONLY THING TO DO is take it off and adjust it to make sure the valve is completely closed.

Also, is the gasket leaking?

Did you over tighten it and warp the casing (very doubtful)

iv never touched it before... SC went off and got ported maybe it got bumped in shipping... but iv never done anything to it...

had a thought... i used to have a boost bypass kit on truck when I bought it... found out it was installed wrong... i took it off and am now back to stock... forgot to ever do it again... could that be a problem??? should I install one of those again?? would that cause me to lose 4+ lbs boost??

no the bypass only "bypasses" boost when no load is on the engine. At WOT it is closed shut and all boost is forced to the cylinders.

I hope you have those labeled backwards, because the black "tab" is supposed to be resting on the set screw when the engine is off. When you crank the truck it should rise, then close back down to the set screw while under throttle.

I would unplug the electrical connection. leave the vacuum lines in there stock position. it will not throw a hard enough code to cause any problems. I had my "boost bypass" hooked up wrong at one time, and it was causing me to lose 2psi. at wot.
