There's a thread around here with pix, but basically you just pry off the trim behind the interior handle, and there's a screw behind it, pry off the trim beside/behind the window, and there's a screw behind it, then pull up the window switch plate, and stuff it back in the hole. Then just pull up on the whole panel then out. At least that's how it is on my '03. I'm not sure about the earlier design (97-98), and if you've got manual windows you'll have to pull the cranks instead of the the switch plate... also, if you have lights in your doors, be careful when removing the panel, and don't forget to unplug the bulb...

Don't remember the post but this might help.
Front Door Trim Panel
Removal and Installation
All vehicles
1. Disconnect the battery ground cable.
Regular Cab and SuperCab only
2. Remove the front door trim moulding.
All vehicles
3. Note: Use the notch to remove the component.
Remove the interior door handle finish panel.
4. If equipped, remove the window control switch bezel.
1 Lift the front of the window control switch bezel.
2 Disconnect the electrical connectors.
Regular Cab and SuperCab only
5. If equipped, remove the window regulator handle (23342).
1 Separate the handle cover from the handle.
2 Remove the screw.
3 Remove the window regulator handle.
4 Remove the spacer.
6. CAUTION: Do not pull directly outward on the front door trim panel, as damage may occur.
Remove the front door trim panel.
1 Remove the screws.
2 Lift and remove the front door trim panel.
SuperCrew only
7. CAUTION: Do not pull directly outward on the front door trim panel, as damage may occur.
Remove the front door trim panel.
1 Remove the screws.
2 Lift and remove the front door trim panel.
All vehicles
8. If equipped, twist the courtesy lamp bulb socket one quarter turn and remove the socket from the front door trim panel.
9. To install, reverse the removal procedure.
*I think the most important thing to remember is when it says to lift the door panel it means straight up, don't pull the panel towards you. You'll break the tabs off of the panel. *
