This is my "theory" on what it is.....
It can be any noise that resonates into the cab at a particular rpm or speed. Usually linked to the exhaust system in some manner......
It can be either from the muffler itself or a vibration from the exhaust system. Usually occurs at the same rpm/speed because that's the time the noise makes it's specific 'pitch' that your ear picks up and drives you crazy!! I'd imagine some people have different levels to where this certain pitch affects them, but that rpm range seems to be about the most popular...
In my case, the only time I'll get what I call a "drone" in my ears is when I'm towing my travel trailer and going about 50 mph in third gear which is right around 2000 rpms. Go slower or faster and it's not there. Hit it just right and you can really hear it inside the cab. It's the only time it does it. Running empty and going the same rpm/speed does not produce the same 'pitch' or 'drone'....

Best I can describe it...
Technically, is a resonance frequency of sound where two similar sounds waves are only slightly out of phase. The Peaks and valleys of the sound waves are only slightly apart.
In most cases, this is a pleasant sound like a barber shop quartet...
However when the waves go farther out of phase the sound starts to get a WAH..WAH..WAH..WAH thing going. Is makes you hear an unplesent buzzing or maybe more accurately a ringing sound. Not unlike a police or warning siren ( designed to be annoying ). It gets louder and softer and a specific time interval.
It is not the pleasing V8 rumble that we all like so much....
Anyway, that is the best I can describe it...
P.S. I notice that mine does this more after the exhaust gets hot...
