P0054, AND P0581 codes

I'm not sure if this is where I'm supposed to post this but I've been getting these error codes and can't seem to get them resolved.

P0054 : H02S Heater Resistance (Bank 1 / Sensor 2). This is an odd scenerio so I'm going to try and break it down into bullets to make things easier to read / understand.

1) I had a set of headers / high flow cats / Y installed approx a year ago and initially had no codes.

2) Two weeks after the install the bolts fell out of my exhaust between the Y and mainpipe. The (sensor 2 is in a bung approx 6" behind junction). I bought new bolts / lockwashers and tightened it down. Code dissapeared for a few hundred miles and came back on.

3) I replaced the O2 sensor and the code went away

4) A month later it separated again at this junction and again I got the code. (No idea why that section was separating). Seems like there's enough play to account for some movement.

4) Once again picked up bolts but this time I double nutted them. The code DID NOT go away this time around however, and I refused to pay another $90 for another sensor so I drove it until I needed my emissions test.

5) Replaced my O2 sensor once again, cleared the codes, drove for a few hundred miles, passed my inspection....And 500 miles later the code is back again. It's hard to trace the wires but I see nothing physically wrong with the casing that I can see. Replacing the O2 sensor seems to be a temp fix though so I'm trying to get to the root of the problem to get this taken care of once and for all. Looking for any suggestions or thoughts for things I can do on my own without putting it in the shop.

2nd Code: P0581 : Cruise Control Multifunction Input A Circuit High. Maybe it's a cooincidence but these two codes seem to appear at the same time and I don't know if it's possible they could be related.
