All right bro, here ya go. Instructions straight from my Haynes manual..

Removal
1. Set the Parking brake, then place the shift lever in N
2. Remove Center console side panes and top console shift cover
3. Separate the shift cable end from the shift lever usina a screwdriver
4. disconnect the shift cable from the shift assemble mounting bracket.
5. Disconnect the shift lock solenoid electrical connector.
6. Remove the 4 bolts and remove the shift lever assembly from the floor.

Installation
7. Installation is the reverse ot the removal procedure

That is as detailed as this book gets.

No idea! You can always take the lid off and use the shift-interlock override to take care of it - so it's driveable, just ugly.

Walk to the nearest auto parts store and buy a repair manual. Never attempt a task unprepared.

Lever in the front huh??? Well I dont think the spring would go on the top as you have it picture. Hmmmm, why did you take it apart in the first place???? I hope you learned your lesson, pay more attention next time! Hope someone can help you out, its probly going to be something stupid as hell that you will kick yourself in the *** for not seeing on your own.
