I got my kit from a local Houston dealer. I do not know the original manufacturer. I paid $250, but I think you can do better. At the time, kits for 04s were hard to come by.

If you know a good truck toy shop, you may be able to buy just one piece of the wood and trim/cut it to fit. I have the same issue as you with the extra hole in the dash after replacing the factory stereo. But, I have decided to leave it as it is, at least for now.
