There is 2 screws holding it down. You need to remove the facia around the stock CD player, and the CD player (i think) to remove the screws holding it in. Maybe you can get around them without removing the cd player.
search for removing stock cd player, and if you follow that, you will see the screws pop right out at you.
(This is how its done on an 04 SuperCab XLT)
Well I cant imagine the rubber insert being installed differently in different model trucks, but I have the FX4 and mine just pulls right out. Theres 2 little rubber nipples underneath it that just pop in and out.

Thanks for the replys. I'm going to try pulling on mine some more. I tried to just pull it out last night and it seemed to be in there pretty tight. I thought I'd ask before I forced it.

I think there's a rubber boot within a plastic tray. To remove the rubber, just pull real hard and it'll come out, but you might never get it back in right.
To remove the entire tray, first remove the fascia around the cd player and climate controls. Then you'll see two screws at the top holding it in, I believe they're 5.5mm, but might be 6.5
