First guess would be the resistor back....the voltage for the blower motor goes through it to control the speeds, but it is usually bypassed on the HI setting. It could be burnt out.
Or else the blower motor brushes are worn/corroded badly enough and it cant get enough voltage unless its getting a full 12v to turn on.

Do you have the auto climate/fan contol?
If you do the electronic resistor control is commonly failing. The contacts inside the controller heat up enough to melt the solder and loose connection. Don't replace the $65 part. Open it up, check the connections for continuity and solder the open connections back together. This will continue to happen every couple of years on most auto climate contol Fold/Lincoln/Mercury vehicles. To prevent this from occuring, only operate the fan on a high setting so the resistor doesn't heat up. (not a realistic option for most)
If you only have a manual control fan on the climate controls your resistor is bad. If the brushes were worn enough to prevent the motor from turning on low and medium it wouldn't turn on high either.
It could also be switch itself, but highly unlikely.
