Jolts At A Stop And When Accelerate

Hello. I have a 02 5.4 F-150 FX4 SuperCab. Its actaully been a couple of weeks now that this has been happening, but sometimes when I come to a stop, my truck will jolt and feel like it wants to take off forward unless I push on the brakes hard, then it will subside. Also, when I accelerate from that stop the transmission will jolt like the gears arent catching or something? This usually only occurs when I am driving fast or come to a fast stop. If I take it easy usually the jolting wont happen.
I dont know much about cars but very much want to learn. I have not checked the transmission fluid yet, I'm planning on doing that later today, I was wondering if someone can give me some suggestions within the next couple of hours while Im at work.
