Deep Grooves in Disc Brakes. What causes? What to do?

I've developed deep groves in my rear Disc rotor. While this isn't actually for my ford (its for my motorcycle), the situation should be identical to if it was true with my ford. A visual inspection of the rear brake pads looks OK. we are talking maybe 1 CM+ deep grooves. Clearly too much to refinish the rotor. I'll have to replace it.

What could be causing it? or is it only going to be a bad brake pad? Should I replace the pads and the rotor or is there something else I need be wary about or inspect?

thanks for any help!

From the sound of it you got some little pebbles stuck in that thing. They clip to the pad when the pad is warm and absorbent (kinda). This happened to my dads 99 tauraus waggon (The Jelly-Bean). Anyway since its a motor-cycle its impossible to do this but if you know you got somethin' stuck nextime drive fairly fast in reverse and brake-check (or emergency stop). The pebbles should fall out.
