Vaccum leak on 04 5.4l

Vaccum leak on 04 5.4l
My 04 5.4 has been throwing the following codes: p0171,p0174,p1000,p2195,p2197
I've boiled it down to a vacuum leak located behind the intake region on the passenger side. Can someone please help me figure this out! I've done replaced intake gaskets and driver side pcv vacuum lines. I've been told there is a vacuum line on the back of the engine, is this true and what is it for and how can I fix it? My truck won't keep a steady idle. Idles anywhere fro 400-1400rpms and it fluctuates.
