191K maintenance checklist
I'll spare you the details of why, but I just bought an '05 Supercrew 4x4 5.4L with 191,000 on the clock. The goal here is to make her last another 20,000 or so. New plugs, air filter, fuel filter as well as an oil change are happening tonight. Although I'm not sure which oil I'll be going with. I usually use Mobil1 full synthetic, but I've never had a vehicle with this many miles on it. Anyway, I've got several other maintenance type things in mind to do, but I wanted everyone else's suggestions as well. The truck runs and drives excellent with the exception of a small intermittent vibration in the idle, both in gear and out.
I'd pull and clean the TB/EGR/IAC. Radiator flush. Tranny fluid/filter change. Change fluid in transfer case/rear diff/front diff. Personally I'd change the upstream O2's also.
But that's just me. If it runs great and your goal is only another 20k why do anything other than an oil change? That truck could probably make it 20k more without being touched.

191K Isn't as bad as you think. I'm running 267K (essentially highway) and I pass emmissions test easily.
When I bought her I think that the mileage was around 78K.
My oil has been mostly Quaker State Full or Synthetic Blend 5W-30, and I replace mostly all of my underhood rubber parts and assemblies yearly or bi-annually. Replace the fuel filter often; since I have been a victim of some bad contamination problems. Good Luck

Thanks for the replies. I was leaning towards the tranny flush, transfer, front & rear diff. lube as well. You guys are probably right, the truck would probably make it another 20K just fine, but I just cant stand to drive a truck that will [I]probably[I] perform when I need it to. At least taking these preventative maintenance steps will give the truck attention in its vital components. Thanks again!
Leave the plugs alone unless it starts to miss. You have the engine that is subject to stuck and broken plugs.
Change the oil with cheap Motorcraft 5w20 semi-synthetic and a Motorcraft filter, and go ahead and change the air and fuel filters, also Motorcraft. Throw a bottle of Techron in the gas tank.
If you do the other fluids, do it right. Have the tranny done with a fluid transfer machine, Mercon V ATF. Transfer case also takes ATF. Front diff is 80w90 conventional (75w90 synthetic acceptable), rear diff *requires* 75w140 full synthetic, and Ford friction modifier if it has limited slip.

thermostat. If this sticks and you run hot for a while you will damage the engine. I drill 2 holes in the thermstat so the truck runs a little cooler and will not overheat if the thermo sticks.
I'd say if you know what oil was used in it before, stick with it. If its worked well thus far, why change? (This coming from the guy whose truck has had Mobil 1 since it was first purchased...)
Avoid spark plugs unless you think there is a problem or your mileage is real low. Like mentioned above, they are prone to breaking off in the block, and then you have a whole 'nother problem.
Check your air filter, too. That can easily rob you of precious MPG, as I have learned first hand. A fuel filter change never hurts, either, and its super cheap.
