Is it time for plugs? Changed fuel filter or cleaned throttle body?

It's my understanding that you clean it. I think the dealer even has a service for it.

If you are filling at the same station, change brand and station. I just went thru this with my 4.6. My mileage fell to an all time low of 13 mpg on a 4.6 SCrew that normally gives me 20-24 hiway. I found that apparently the usual station isn't moving gas quick enough and the ethanol was losing it's octane rating that is in the federally mandated formulation. The 87 octane I was buying ended up being a little over 80. I'm even more surprised it started and ran on it. But it was extremely sluggish and fuel mileage seemed non-existent until I changed brands and station. Until they quit using ethanol, and the science says they will have to quit, we'll have to put up with crappy running engines.

I was going to say that there might be a problem with the fuel, and octane, but din't think about the ethanol aging.

Find a local brand that runs well and gets good mileage, then stick with it.
