I have a 2004 Lariat 5.4 4x4, and have had extensive work done on the motor and drive train, including rebuilding the Right head, and replacing the tranfer case and transition. I do not use this truck as a work vehicle, mainly a daily communter and dump runs on the weekend. I have owned fords my whole life but have never had this kind of trouble with a truck with under 75000 miles. Any one else have any similar issues? Should I worry?

Have you owned this truck since it was new? How many heads were rebuilt and what was the cause(s)? Was any of the work done under warranty? There are many details/history that we don't know so hard to say what you should expect or if you should really worry. If you're maintaining properly and continue to have major issues, obviously there's cause for concern as these trucks are generally very reliable.
unfortunatly the 2004 model year doesn't seem to be a good one. First year for the 3v engine the electronic throttle body and all. I wouldnt own a 04-07. They seem to have allot of issues. Cam phasers on the head? that seems to happen allot that year..

To answer your questions: I am the original owner, and have had the truck serviced by FORD for all scheduled maintenance except belts hoses and filters which I have done my self. All repairs have been also completed by ford under an extended ford warranty (thank God I purchased it). They rebuilt the right head reporting a bad phaser. At the same time they replaced the left exhaust manifold to correct the low RPM surging. The warranty has 2500 miles left and I am getting an uneasy feeling. The truck is bone stock other than a drop in K and N filter.

Sounds like the extended warranty was a good purchase on your part. I got one & only used it for replacing the moon roof motor, which actually turned out to be pretty pricey. If you sense that your truck will continue to cause you problems (and you want to keep it) another extended warranty might be worth it for you (especially if you don't want to do work yourself). On the other hand, a lot has been done and hopefully all the bugs have been worked out. I don't like extended warranties but sometimes they are worth it. Your's MIGHT be case for (another) one. As Jet mentioned, 2004 was the first model year for the 3 valve engine and both the 2004 & 2005 model years had a fair share of problems, at least a lot more than the previous model years. Was most of this major work done recently?
