Engine Hesitation @ 40mph? Help!

At 40 mph, w/no load, noticed a hesitation while mildly accelerating.
Have 04 lariat 4x4 screw w/ 84k miles and well taken care of. Just changed oil/oil filter, fuel filter, air filter and using better grade of gas @ notible gas stations. Truck runs great at idle, take-off and at highway speeds too. Can anyone assist me with some input with this topic? Thanks in advance.
