How do you know its misfiring in every cylinder?

Also, how will cleaning the plug wires help any, I suppose to mean you cleaned the contacts? I would go ahead and replace the wires since nothing else has helped.

My 4.9 was doing somthing simular I found a problem with the seal between the throttle body and manifold... start looking for a vacume leak.. could be the issue..

Is the timing good?

My issues are 100% solved yet but that leak on the TB was a major part of them.
