These errors seem pretty common and searching these clues in the forum give the following answers - but all lead to many more questions. Lets try to get a central discussion to fix this. I'll start!
What's worked before to fix this? Here's the combined forum suggestions.
1) Rotten PCV vacuum hose and elbow.
2) MAF sensor dirty or defective.
3) Possible upstream O2 sensors.
4) Fuel filter
I suspect if these errors show up at the same time, wouldn't it make sense it's not likely the O2 sensors. What are the odds that both fail at the same time?
Who has gotten the MAF fix to work? Has anybody acutally fixed this by replacing the O2 sensors, and fuel filter?
I had the vacuum problem - that resolved the errosr for a few weeks, it's back. A new fuel filter did nothing to change the current codes.
I'd like those who have actually resolved this for over 100 miles to chime in!

Yesterday I had some items replaced:
Plugs (Autolite DP APP104)
Wires (Motorcraft 8mm)
PCV valve
Fuel filter
It has only been one day, but everything is WAY better.
This thing has never run better. I bought it at 88k, and I'm at 93k now.
The idle is smooth, and no more misses.
I had bought front o2 sensors, but my manifolds are so rusted that my mechanic (close buddy) said to wait to do them. I'm going to do manifolds/o2's/EGR valve/EGR-tube (to manifold) next.
Prior I had:
Cleaned throttle body/EGR ports
Cleaned IAC
Replaced air filter
The truck is really coming around, but it's an investment for sure.
Soon I may need to drop the tranny and do the rear main seal, and tranny output shaft seal

Ran 100 miles on the freeway tonight, and the tune-up still feels great.
Before the tune-up I was also getting insufficient EGR flow, and bank 1 & 2 lean codes.
Funny but.....these few items really made a difference. No CEL so far either.

So what would you put your finger on? Did the throttlebody or EGR make much of a difference? I haven't gone through the list as completely as you(no plugs and wires yet) but I have also noticed a difference. When she starts the engine just jumps to attention.
I know plugs are hard to get to, but did you have any trouble getting them out, do we 98'ers need toworry about stripping the threads?

TB/EGR had almost no effect, inspite of being 100/60% clogged.
Cleaning the IAC seemed to help my idle, but I think the above items helped the most.
Yesterday I ran another 100 miles with two Dirt Bikes in the back, and four people in the cab. The engine pings more than I would like. I'm hoping my up-coming *manifolds/o2's/EGR valve/EGR-tube (to manifold)* project helps that.
Still, I'm much happier.
I'll be dropping a 12-pack off to my mechanic tonight
