93 F150 5litre->nasty sound

93 F150 5litre->nasty sound
Hi there
I have a 93 F150 with the 5 litre engine, just last week or so i have noticed a knocking sound like lifter clatter coming from up front, this only happens when the tranny shifts from 1st to 2nd(between 40 and 50 kph).
This does not happen all the time, i have heard this sound before from a few other vehicles and the end result wasn't pretty, could it be the top end going or bottom end going?
The engine only has 165k kilometers on it and i take care of it religously.
This truck has been really good to me and if i can i would like to save the motor if it isn't cost to cost me and arm and a leg.
