p0171 and 174

So i plugged into the mac tools scanner a few weeks ago and had a lean condition in bank 1. After looking around on here i decided that i should clean my MAF as it has 96,000 miles or so. Cleared the trouble code and check engine light was off for a week or so and came back on. I plugged back into the scanner and now had lean condition in bank 1 and 2. I changed the fuel filter and had the light come back on 3 days or so later. I just got done checking vaccum hoses and came up with nothing. I also checked operation of PCV valve and have a vaccum at the valve. The next question is that just because i have a vacuum at the valve does it mean that its working properly?? Also should the hose at the top be able to swivel, and how tight should the PCV valve fit into the valve cover???

Put that scanner to use and check freeze frame data, if it happens w/ a cold engine, replace the upper intake gaskets. Also, there is no need to clear codes every time, just check to see if short trim is removing fuel to bring long trim back to zero if it isn't, it's not fixed and vise versa for a rich condition.
