I had a 2002 that did the samething. Turn yourself upside-down on the seat and look up under the dash at the cable that runs from the shifter. The cable has a mount close to the steering shaft. Mine broke at the mount. I used a zip tie to pull the cable tight from a frame point.
Either that or your cable broke completely.
Wish I could help more, but I no longer own the truck.
If you replaces the bushings and pin, and it still does it, then something came apart inside the transmission.
Only fix is a rebuild/teardown or reman tranny.
I suggest finding a reputable tranny house in your neck of the woods if you want it done right.

I had this happen on a car. It turned out to be the clevis pin that connects the stick shift to the tranny. In my situation, a cotter pin had broken and the clevis pin slipped out of the stick shift. Easy fix, hope that is what's wrong with yours. Lift off the stick shift boot and shine a light in there, move the stick around, you'll probably see what's wrong. You may even see the clevis pin sitting in there somewhere and all you will need is a new cotter pin.
