Manual Trans.

I have a 2000 f150 xlt with 215000 mi. on it never changed the trans. fluid. Do you think it is time? Never had a problem yet know on wood. Should I replace fluid or just add if needed?

I know most will agree to change the fluid but personally I would not. Murphys law states if you mess with something that old that isn't broke it. It will be.

On a manual, you don't have to worry about any hydraulic systems (like the valvebody and servos in an A/T) so you're not going hurt anything there.

With 215k on it, there's probably a good bit of synchro dust and maybe a few other metals flakes from the bearings or gears mixed in with some pretty well broke down oil. Mix in a little water from condensation over the years. Yep, I would change it.
Same with the diff. fluids if they haven't been done either.

BUT...make sure you can get the fill plug loose before you crack the drain plug to drop the fluid!
First time I dropped the tranny fluid on my 90 Nissan (at 165k) the fill plug was froze in solid...didn't find that out until after I drained it and I wound up having to fill it through the shifter.
