Copenhagen's truck is the model for a 4 inch. Looks awesome.

I'd say 35x12.50s on a RCD 4.5" lift. I had the Rancho lift on my 2004 F150 and it was a good lift. I never had a problem with it and I beat the snot out of it. So whichever lift you pick, you'll have zero problems. RCD is a little bit nicer of stuff though, it's also more money.

x2 - RCD>Rancho for overall quality (especially in the ever important shock category) but keep in mind the RCD will have to be pieced together in order for it to look level @ 4" as they supply you with a 5" block. You could go with an Auto Spring 3" block off of eBay for an extra $100 and it'll look nice there. I'm not knocking Rancho but it's not the same as RCD. Try and go with a Rancho sans the shocks and get some Bilsteins for it and it'll be like night and day...
