Wow. Tough situation, read the entire thread. I ran my tcase on hardly a quart of fluid for a long while, at least a month. Point out the significance of everything from the 10 miles after leaving the dealership, etc. If the bearing was going to go due to lack of lubricant wouldn't there be evidence elsewhere (shavings), and wouldn't it be visible to the man changing the rear diff. This is a tough situation for both you and Ford, but is pretty clear in my opinion Ford is at fault.

This definitely sounds as though you will be going to court to get your dealership to cover it. It does sound as though your insurance company's adjuster believes it was the dealerships fault. If that is the case, and they cannot get the dealership's insurance to cover it, then they should be willing to repair or replace the truck at current value and not increase your rates.

Worst case scenario - your insurance company fixes/replaces the truck and raises your rates, right? I still believe that the increase in rates will be far less than court costs, lawyer fees, expert witness fees, and headaches if you take it to court.
