PA 3" body lift - HELP

I am missing a bolt pack with my Performance Accessories 3" body lift kit. This is kit #863. Can anyone tell me what type of bolt I need. Not the size, but the type of head on the bolts. Slotted flat head? Standard? I know they should be at least Grade 5. I just don't know the type. Thanks.

If this is for any of the body mount bolts they're all standard hex head. Make sure you get grade 8, NOT grade 5. FYI, in case you didn't know the bolts are metric threads. I had to get a couple longer bolts when I did mine (drilled out the wrong mounts by accident). I was able to find them at Lowes, though its the only time I've come across anything stronger than grade 5 there.

Yes, finding the size hardware I need has proved more difficult that I expected.
I need bolts in size 12mmX1.25X180mm, 12mmX1.25X200mm and 12mmX1.75X140mm.
No luck yet.
PA wants $107 for the bolt kit. I may not lift my truck after all.
