Undercover makes a nice one that is as close to flush as you will find in a hard cover, and is definately weather tight.
I do not have a picture but you can go online and do a search and find their sight.
I just installed one on my 2004 Supercrew, and had one on my 2001 Supercrew. They sell in the $600.00 to $700.00 range depending on the dealer.

I have the Mountain Top (www.mounttop.com) and love it!
It can be ordered in several configurations. I have the original one. Not only does it keep stuff dry in the bed, you can put stuff on top w/o worrying about scratching it, etc.

Pace Edwards makes a nice roll top that is almost flush. I have one on my truck and love it. The down side is you do lose a little bit of the front upper portion of your bed. It does not affect plywood or stuff like that but it could be a problem if you are hauling ATV's because you would not want to slam them into the box.
