Code 628 excessive converter clutch slippage

I have a 91 ford f150 4x4 5.0l with the E40D tranny. I have this code coming up even after getting the tranny rebuilt and a new torque converter. I took it back to the tranny shop they checked it out and said the only thing they could find was a engine coolant temp fault. I have replaced the ect and I checked the voltage from ecm to the ect I got 4.62v according to my Haynes manual it should be aprox. 5v. Is this in an acceptable range. Is there any other ideas of what would cause this before I go back to the tranny shop. I appreciate any and all input on this issue.

Did they or did you reset the computer/clear codes by unhooking the battery for about 10 min.?

Been unhooked so many times I've about wore the cable end out I found that the tranny shop decided to use my old solenoid pack so therefore I was still having that problem took it back to shop they are currently taking care of it after a few choice words.
