One of my outside power mirrors needs to be replaced. Can the glass itself be replaced, or does the entire "pod" assembly get replaced?

If just the glass can be replaced, where is the best place to buy it? Ford or a glass dealer?

You can replace just the glass.
If it's a MUTH mirro, you can get them here
or from MUTH. If it's factory ford, You'll need to get it from the dealer.

Yes, just the glass can be replaced, I had to do the same thing just a few months ago on mine. Don't even try to find one anywhere else, Ford was cheaper than the junkyards with the replacement glass, and it only took them a few minutes, and it has worked perfect ever since (knock on wood). With labor, the cost for one mirror was $125, and this was a deal compared to online junkyards selling them for upwards of $200. Good luck on getting everything done.
