there was some kind of web site you could do it with. I did it but dont have the link anymore. You have to sign up for a potential investor's package. A big packet will come in the mail, one of the sheets is a registry for a complimentary x-plan number.

A post about the X-plan. You coud just print out one of the forms and fax it in. Does anyone have a form they can scan in or give the link again??
