I've been suspecting the tune all along; but, didn't want to go there without checking eveything else.

I'm waiting for a tune now from another tuner to test out this theory. Apparently, I'm pretty far down on the waiting list for a retune though. Still open to suggestions.
The dealership said mine was carbon as soon as I called and explained the problem. They had seen a couple other L's with the same issue. This is a common problem with the 5.4 in general. We have an all Ford fleet at my work and everything with a 5.4 that we have has had a carbon builtup issue at some point. Also these are not treated gently, these are EMS vehicals and are pushed pretty hard, probably harder then most here push their trucks.

The decarb didn't work; but, running a way too lean tune for a few miles seems to have cured the problem. (Read bad detonation on the new tune.) I put the old tune back in, and no more codes for almost 3 weeks now. Looks like I needed extreme measures to get the carbon burned off.
